Eileen LaFavor, Administrator
Hello, my name is Eileen LaFavor. I have been the Administrator of the Baldwin Care Center and Birchwood Apartments since June of 1988. I am originally from Eau Claire. I received my Bachelor's Degree in Health Care Administration from UW-Eau Claire in 1980. I have worked in the health care field since 1980 in the capacity of Personnel Director and Administrator at several other skilled nursing facilities in Wisconsin, prior to moving to Baldwin. I currently live in rural Baldwin with my husband and teenage daughter. My son lives and works in the Madison area.
My responsibilities as the Administrator include directing and managing the overall operation of Baldwin Care Center, Inc., including the Baldwin Care Center and Birchwood Apartments. During my tenure at the Baldwin Care Center, I have had the opportunity to be involved in several building projects to expand the services on the campus. I am very excited to be involved currently in the process of the creation of a new nursing facility and assisted living setting. The new skilled nursing facility will create a homelike environment of the residents of the Baldwin Care Center, providing person-centered care to each resident. The recent addition of a Community Based Residential Facility (assisted living setting) creates a continuum of services on the campus ranging from independent apartment living in the Birchwood Apartments, to Birch Haven, an assisted living setting, to the skilled nursing services in the Baldwin Care Center.
Long term care has changed a great deal since I began in 1988. I look forward to the challenges of meeting the needs of residents in the future.
Linda Zevenbergen, Birchwood Apartment Manager
My name is Linda Zevenbergen. I have been employed with Baldwin Care Center/Birchwood Apartments since 1994. I enjoy my job as Apartment Manager. I have been in long term care for over 30 years. I started out as a Certified Nursing Assistant. I worked as an assistant Activity Director for 10 years. I now work as an Apartment Manager. I can look back and recall that there have been many changes. I wouldn't do anything differently; it has been rewarding working with the elderly.
My job involves making the tenants feel comfortable in their new home. The goal is to have the tenants stay as independent as possible. There are activities scheduled at least twice daily, which include once a month lunch outings, also socializing with area assisted living facilities. In addition to opportunities for involvement in activities and socialization, the facility provides many supportive services such as assistance with medication, bathing and therapy services, just to name a few.

Tammy Casey, Birch Haven Manager
Hello,my name is Tammy Casey. I am the Nurse Manager of Birch Haven Assisted Living, which is connected to the Birchwood Apartments and Baldwin Care Center. I graduated from Chippewa Valley Technical College in River Falls with an Associate}s Degree in Nursing in 2011. I worked in Community Based Residential Facilities (CBRF), Residential Care Apartment Complexes (RCAC) and Long Term Care facilities as a Nursing Assistant/Med Aide while attending nursing school. After graduating I worked as a RN for two years in a Long Term Care Facility.
My role as Nurse Manager is to oversee and supervise CBRF personnel and the overall operation of Birch Haven. Some of my responsibilities include: maintaining compliance with state and federal regulations, communicating with families and healthcare professionals in regards to resident well-being, overseeing medication management, and performing resident assessments to establish accurate levels of care.
I have a passion for helping others, especially the Elderly, and look forward to being the Nurse Manager of Birch Haven.
Denise Hague, Director of Nursing Services
Hi, my name is Denise Hague. I live in Spring Valley, WI with my family. We raise horses and currently have twenty of them. I enjoy the outdoors and spending time with my family.
I have been an employee of the Baldwin Care Center since 1986. I worked six years as a nursing assistant, then in 1992 went to Southeast Technical College and received my nursing degree. I 0orked as a Licensed Practical nurse for twenty more years before deciding to go back to school and earn my associates degree and become a registered nurse. As luck would have it the Director of Nursing position here at Baldwin Care Center became available about the same time. I finished with my schooling. So I am very happy and excited to say that I am the new Director of Nursing here at the "new" Baldwin Care Center. Some of my duties include supervising nursing staff, hiring new staff, and most importantly making sure our residents are happy and safe.
I have always very much enjoyed working at Baldwin Care Center and I am looking forward to serving the residents that live here and their families. Please stop in to visit, or to tour our new facility anytime.
Amy Roemhild, Social Services Director
Hello, my name is Amy Roemhild, CSW. I have been the Director of Social Services at the Baldwin Care Center since May of 1998. I have a Bachelor's Degree from the University of WI-Eau Claire in Social Work and have pursued my Master's Degree at the University of WI-Stout in Human Development and Family Studies. I live in the community of Baldwin with my husband and three children.
My role at Baldwin Care Center is to ensure and enhance the psychosocial well-being of the residents. Whether staying on a long term basis or for short term rehabilitation, I work with families and residents to establish open communication and person centered goals in their care and life at the Baldwin Care Center. Times are changing and the Baldwin Care Center is committed to honor each resident individually with resident's choices and rights being a high priority in a home like environment.
Some of my responsibilities as the Director of Social Services are to assist with admission and discharge planning. I am available to assist with questions related to Medicare/Medicaid and referrals to community resources as needed. I assist with adaptive equipment and clothing needs as they arise. I continue to assist in educating staff to ensure our residents well being is the top priority. I am available to give a tour and discuss the needs of residents and family before staying at the Baldwin Care Center. We strive to make the Baldwin Care Center a "home" away from home.
Danielle Lausted, MS RD CD, Dining Services Manager/Registered Dietician
Hello, my name is Danielle Lausted. I have been the Dining Services Manager and Registered Dietitian at the Baldwin Care Center since October of 2014. I received my Bachelor of Science Degree in Dietetics in 2001 as well as my Master of Science Degree in Food and Nutritional Sciences at the University of Wisconsin-Stout in 2003. I have worked in the nutrition field since I graduated and for the first 10 years of my career, I worked as a Clinical Dietitian for Surgical and Medical Weight Loss at the University of Minnesota Medical Center in Minneapolis, MN prior to starting here in Baldwin. I live here in Baldwin with my husband and two daughters.
My role is to ensure that residents are meeting their nutrition needs for health maintenance. Myself as well as the role of the dietary staff, is to ensure that meals are nutritious and tasty and provide the residents with a wide variety to meet their personal expectations. We make sure that residents are provided with choices at each meal and try and make the meal times flexible to allow residents to eat at a time that they prefer. Snacks and beverages are available 24 hours a day in the country kitchens located on each side of the nursing home. Coffee is served daily in the dining areas both morning and afternoon, and we encourage families and friends to join the residents for coffee time, a meal, and special parties.
Paul Auckland, Environmental Services Director
My name is Paul Auckland, Director of Environmental Services for the Baldwin Care Center and the Birchwood Apartments. I have been working for the Care Center since 2007. I am responsible for the oversight of the housekeeping, laundry and maintenance staff.
All of the members of my departments work very hard to make sure our residents needs are met, and that their stay here is as trouble free and as comfortable as possible.
We do our best to accommodate the wishes of our residents to make their rooms as homelike as possible, bringing in their furniture, pictures, and other belongings.
Dawn Fern, Life Enrichment Coordinator
Hello! My name is Dawn Fern and I am blessed to be the new Life Enrichment Coordinator at the Baldwin Care Center as of April 2014. I have lived in Baldwin for 48 years and have been in the activity profession for 22 years. I have a passion for working with our elders as they have so much love, knowledge and wisdom to share. In 2007 after classes and training geared toward the activity profession
I became Activity Director, Certified through the National Certification Council For Activity Professionals and then in 2012 Activity Professional Board Certified through the National Activity Association Credentialing Center.
I am married to my high school sweetheart Duane and we have a daughter Allison, son-in-law Mitch and two grandchildren, Marley, age 6 and Crosby, age 3. We have lots of fun with our grandchildren and enjoy spending time with our family.
My goal as Life Enrichment Coordinator will be for each one of our residents to have an excellent quality of life, choosing how they spend their day, where they want to go, what they want to do and when they want to do it. Empowering our residents is what Baldwin Care Center is all about and I am proud to be a part of their team.

Sheila Mikla, Office Manager
Hello, my name is Sheila Mikla. I am the Office Manager of Baldwin Care Center. I have been employed with the Baldwin Care Center since October of 2016.
I grew up in the Spring Valley area and graduated from Spring Valley High School. I have lived in the Baldwin/Woodville area for the past 30 years. I live on a hobby farm and we raise beef cattle. I love spending time with our children, riding motorcycle and embroidering.
I have several responsibilities at the Baldwin Care Center. I do Medicare & Insurance Billing. Private Pay Billing, Census Reporting, Payroll, Answering phone calls & greeting friends and families as they enter our beautiful facility.
Prior to coming to Baldwin Care Center, I was a school bus driver for 16 years and certified drug & alcohol screener for 5 years. I currently own Wicked Stitch Embroidery and in my spare time I work in my gazebo doing embroidery.
I am fairly new at the Baldwin Care Center, but have absolutely loved this new journey in my life. I enjoy working with the staff, residents and their families. We are a great big family here at the Baldwin Care Center. I would be happy to answer any questions that you may have. If I cannot help you, I will find someone that can.
